迎接中国口腔医学发展的新时代——专访中华口腔医学会会长王兴教授

　　“改革开放30多年来，国家不断发展，人民生活水平日益提高。在这样的大环境下，中国口腔医学工作者也抓住了这一难得的历史机遇。”中华口腔医学会会长王兴教授告诉博观。在采访过程中，王兴教授向笔者展望了中国牙科未来的发展路径;民营口腔医疗所面临的机遇以及挑战;中国应当如何以全新的面貌，面对已经到来的数字化口腔时代。

　　曾经的落后孕育着巨大的发展潜力

　　王兴教授经历过改革开放以前中国口腔医学发展的落后状况，同时也见证了我国改革开放以后口腔医学大发展的历史进程。说起这些来，他如数家珍，一组组数据就像刻在了他的脑海里，信手拈来。

　　“1978年以前，全国仅有那么6-7所学校的口腔医学系在培养口腔医学人才，偌大的中国仅仅只有5千多名口腔医生，平均5万多居民才有一个口腔医生。”王兴表示，在普通老百姓的眼里“牙病不算病”，绝大多数的口腔疾病患者，并不会定期去看口腔医生，甚至绝大多数的老百姓连基本的刷牙习惯也没有，更谈不上一日两次刷牙，使用正确的刷牙方法和选择使用含氟牙膏刷牙。牙齿缺失甚至是全口没有牙齿成了中国老年人的象征，造成这些的原因是当时的国家经济上贫穷落后，人们健康观念匮乏。人们只为日常的温饱而奔波，口腔健康只能是一种奢望。

　　改革开放30多年来，国家不断发展强盛，人民生活水平日益提高。在这样的大环境下，中国口腔医学工作者也抓住了这一难得历史机遇，在我国大力推动大众口腔健康教育。1989年以卫生部牵头的九部委联合发文，设立了“9.20爱牙日”。全国口腔医学工作者持续不断地开展大众口腔健康教育，每年“9.20爱牙日”的活动主题囊括了口腔健康的方方面面，深入到我国城乡各个角落。这样的大众口腔健康教育极大促进了我国老百姓口腔保健意识的提高，大众对口腔健康的重视奠定了我国口腔医学发展的坚实基础。我们看到了口腔医学教育大发展的局面，看到了口腔医疗机构蓬勃发展的景象，特别是民营口腔医疗机构从无到有从小到大，遍布全国城乡各地，为我国大众的口腔疾病治疗提供了基本保障。

　　现在，全国有180多所口腔医学院校，每年通过国家口腔执业医师、口腔助理执业医师考试的就超过2万人，相当于过去30多年总和的4倍多。一大批具有国际一流设施和水准的口腔专科医院迅速崛起。特别是在北上广、沿海经济发达地区，我国口腔医学的发展水平已经基本达到中等发达国家的水平。

　　但是对于一个拥有14亿人口的大国来说，我们取得的成就还远远满足不了生活水平日益提高的广大人民群众对于口腔保健的需求。和发达国家相比较，我们还存在着巨大差距。我们大约每8千多人才拥有一个口腔医生，更何况我们的城乡之间、东西部地区之间尚存在巨大差距。要使我国老百姓人人都能享受到基本的口腔保健医疗服务，我们还有很长的路要走，这也从另外一个侧面反映出我国口腔医学事业发展的潜力巨大，仍有非常大的市场空间。例如，10年前的第三次全国口腔健康流行病学调查显示，我国各个年龄段的龋病患病率都在90%左右，然而其中接受过治疗的不到10%。试设想，如果其中的50%要求治疗，我们的诊疗工作量将要增加多少倍?

　　民营口腔医疗发展天赐良机

　　改革开放以来的30多年间，民营口腔医疗遇到了前所未有的历史发展机遇。从无到有、由小渐强，已遍布全国城乡各地。近年来更是由于国家政策的鼓励与引导，我国民营口腔医疗的发展驶入了快速道。全国各地涌现的民营口腔专科医院，一批原来工作在公立口腔医学院校的知名口腔医学专家纷纷加盟民营口腔医疗机构就是证明。

　　很多公立口腔医疗机构的同事们担心民营口腔的发展会抢了公立口腔医疗机构的病人资源，挤占公立口腔医疗机构的生存空间;也担心民营口腔医疗机构通过各种措施吸引公立口腔医疗机构的人才会影响公立口腔医疗机构的发展。这些担心确有一定道理，但是已经过去的多年发展历程已经证明，这样的担心起码在现阶段似乎并不存在。我们看到的是民营大发展了，公立口腔医疗机构同样得到了极大的发展，说明在目前阶段尚没有形成民营、公立口腔医疗机构之间的激烈竞争。这是因为中国口腔医疗市场的现实是我们还无法满足老百姓的口腔保健需求。

　　我相信，随着国家的不断发展强盛，人民生活水平的进一步提高，老百姓的口腔保健需求还将进一步增加。我们能够提供的口腔医疗保健服务仍将呈现出供不应求的现状，因此在现阶段，“谁抢了谁的饭碗”的问题似乎并不需要太多担心。但是也许在将来，这样的竞争会发生，会出现优胜劣汰的局面。这就要求所有的口腔医疗机构必须加强自身能力的建设，提高自己的医疗质量和服务水平，依靠自己的人才优势、学科优势在老百姓中赢得良好口碑，也就是赢得未来。

　　突破人才制约瓶颈，创造更大发展空间

　　遇到很多民营口腔医疗机构的管理者、经营者，都会说到一个问题，那就是高水平口腔临床专家人才的短缺，这也成为了制约民营口腔医疗机构发展的瓶颈。是的，在中国口腔医学发展的历史进程中，院校特别是知名口腔医学院校集中了一大批高水平临床口腔医学专家。大学不仅为高水平临床工作的开展提供了平台，同时也为他们的科学研究提供了机会，使他们能够利用这样的平台成为国内知名的专家，享受到民营口腔医疗机构常常无法提供的荣誉与学术地位。因此到目前为止，绝大多数的知名专家还是宁愿拿着较低的经济待遇，在院校口腔医疗机构工作，要他们脱离公立口腔医疗机构完全到民营口腔医疗机构工作似乎并不容易。但是近两三年来，我们还是看到了这样的情况，一些高水平口腔医学专家选择加盟民营口腔医疗机构。随着公立医院医生多点执业政策的逐步落实，我相信会有越来越多的公立口腔医疗机构的医生到民营口腔医疗机构执业。但是仅仅依靠这样的渠道从公立口腔医疗机构挖掘人才是远远不够的，更何况这样做的代价很高，并不是所有的民营口腔医疗机构都可以效仿。那么人才在哪里?如何克服这样的制约瓶颈?就成为了所有民营口腔医疗机构的经营者、管理者不得不认真思考并加以解决的现实问题。

　　“我以为，最现实的解决人才制约的办法是培养人才。”王兴说。

　　事实上，现在所有的民营口腔医疗机构中都有其相应的医疗人员配置，尽管水平参差不齐，一些高水平的临床工作可能尚未开展。但是这些人才是可以通过不断的培养来塑造的，我们可以通过参加高水平学术交流让他们开阔视野;可以通过进修学习、参加各种专门技术的继续教育培训班，使他们在较短的时间内学会、掌握这些技术;还可以与院校的专家联合培养，请院校的专家给与具体的指导与帮助，而且这样的学习与培养要持之以恒地进行。我相信这样做，就会在较短的时间里建立起一支高水平临床口腔医生队伍，一些学科的短板就会在较短时间里得到弥补。一句话，立足于自身培养，人才就会源源不断。没有这样的学习与培养，原来的优势也会随着口腔医学日新月异的发展而落伍。同样的，在公立医疗机构以及院校口腔专科医院中，这样的人才培养机制也是不可或缺的。

　　迎接数字化口腔医学时代

　　进入21世纪以来，数字化、信息化、互联网正在深刻地改变着我们的思维方式和日常生活。同样它们也在日益深刻地影响着、改变着医学和口腔医学的面貌。

　　近年来，口腔医学已经进入了全新的数字化时代。一系列数字化影像技术、三维口内扫描技术、3D打印技术、CAD/CAM技术，已经越来越普遍地应用于口腔医学的多个临床专科，为口腔医学带来了全新的理念和诊疗技术，推动着口腔医学的全面进步与发展。数字化技术将以其精准、高效、易于普及的特点，不断取代我们习以为常的许多传统的口腔医学技术和操作技术。集成的专家智慧将通过数字化技术，进入口腔临床的各个环节。因此我们不得不紧跟这个时代的步伐，在数字化口腔医学时代跑步前行!

　　中华口腔医学会2015年的学术年会以“数字化口腔医学”为主题，就是要紧跟这个时代前进的脚步，及时交流数字化口腔医学技术的最新进展及应用成果;展示一系列国际上具有创新意义的新产品、临床应用的新经验，从而让中国口腔医学工作者在数字化口腔医学时代的起步阶段，与国际同道站在同样的起跑线上，推动和引领中国的数字化口腔医学。大会将组织多个场次的学术会议、专题研讨、继续教育项目以及一系列丰富多彩的展示活动。我相信，所有参加会议的口腔医学工作者，口腔产业界的同仁、朋友都将会有所收获!■


 

　　Meet the New Age of China’s Development of Oral Medicine----Exclusive Interview with Wang Xing, the President of Chinese Stomatological Association

　　Over the past 30 years of reform and opening-up, with the increasing development of people’s living standards, oral medical institutions have begun to flourish in China. In particular, private oral medical institutions have sprung up all over the country.

　　Nowadays, there are more than 180 oral medical colleges in the country. Every day, more than 20 thousand examinees pass the national oral practicing physician exam and assistant oral practicing physician exam, and they are equivalent to more than 4 times general proficiency in the past more than 30 years. In Beijing, Shanghai, Guangzhou, and economically developed coastal areas, China’s oral medicine has reached the level of medium developed countries.

　　But for a large country with a population of 1.4 billion, we have made some achievements, but we still have been far from enough to meet people’s needs of oral health care. We shoulder heavy responsibilities to enable ordinary Chinese people to enjoy the basic oral health care service.

　　Over the past 30 years of reform and opening-up, private oral institutions also have encountered unprecedented historical opportunities. In recent years, private oral institutions began to develop at express speed, and which is proved that private oral special hospitals around the country and a batch of well-known oral medical experts who ever worked in public oral medical colleges join private oral medical institutions in succession.

　　Many colleagues from public oral medical institutions fear that as private oral institutions grow, they will take away patient resources from public oral medical institutions. I believe that as the country continues to flourish, ordinary people will further require oral health care. Oral health care service that we can provide will be in short supply. Therefore, at this stage, we do not seem to worry too much about the situation that who steals the bread and butter. But perhaps the situation of survival of the fittest situation will appear in the future. In this case, all dental medical institutions must strengthen the construction of their own ability to improve their own medical quality and service level.

　　The shortage of high-level oral clinical experts has become a bottleneck that restricts the development of private oral medical institutions. So far, the vast majority of famous experts tend to work in oral medical institutions and colleges. It seems to be difficult for them to work in private oral medical institutions instead of public oral medical institutions.

　　The most realistic way to solve talent control is to train talents. We can organize high-level academic exchanges in order to broaden their horizons and open training courses to enable them to engage in advanced studies and continue to learn various specialized techniques and thus master the techniques in a shorter time. In addition, we also can combine with the experts from colleges to train talents. Moreover, we should continue to carry out such learning and training.

　　In recent years, oral medicine has entered a brand-new digital era. A series of digital imaging technology, three-dimensional intraoral scanning technology, three D printing technology and CAD/CAM technology have been widely applied to quite a few clinical professional subjects.

　　Chinese Stomatological Association takes Digital Oral Medicine as the theme of its academic annual meeting in 2015, which shows that it will keep up with the pace of the times. The associate exchanges the latest advances and application results of digital oral medical technology in time, displays a series of new innovative products around the world and the new experience in clinical application. So that oral medical workers in China are in the initial stage of digital oral medical era, but they stand on the same starting line with the international colleagues, promote and lead the digital oral medicine in China. The meeting will organize quite a few symposiums, seminars, continuing education programs and a series of colorful display activities. I believe that all oral medical workers, colleagues and friends from the oral industry who participate in the meeting will benefit from the meeting.
